When coming in for an office visit, it is often difficult to remember everything you might want to discuss with you doctor. We suggest you bring the following:

• Questions and concerns: Take a few minutes to jot down your issues and questions before you arrive at the office to assure that we can discuss your concerns and needs.
• Bring a list of medications you are currently taking.
• Bring your current insurance card.
